We are seeking a highly skilled and compassionate professional to join our team as a Foster-Adoptive Care Case Manager. This individual will work closely with children, families, and community partners to provide case management services that promote the well-being and safety of children in foster care.
Essential Duties and Responsibilities:
The following are essential duties and responsibilities of the Foster-Adoptive Care Case Manager:
- Continues to build a child welfare system that is safe, secure, and stable; family-centered, connected to the community; and which relies on evidence-based practice necessary to support contract services
- Aacts as a member of a treatment team and gives feedback on client progress
- Makes required home visits to the foster or adoptive home a minimum of two times per month or as directed by the treatment plan
- Makes at least one time per month school visits or as directed by the child's treatment plan
