What are the responsibilities and job description for the Education Transition Specialist position at KVC Kansas Health System?
Position Summary : The Education and Transition Specialist provides independent living resources to Case Managers for youth (17 ) in foster care.
Education : High School Diploma required.
Experience : Office experience and experience with high-risk or foster care youth preferred. Intermediate skills in Microsoft Word, Excel, and Outlook required.
Certification/Licensure : Must be 21 with a valid driver's license and auto insurance.
Responsibilities :
- Build a safe, community-centered, evidence-based child welfare system in Kansas.
- Schedule and facilitate transition meetings for youth aging out of foster care.
- Compile educational info for referrals and attend case planning conferences to explain DCF benefits.
- Prepare and distribute transition packets and assist with educational transitions.
- Support youth in applying for scholarships, Job Corps, and post-secondary education.
- Act as a liaison with schools and assist with medical card applications.
- Train team on Every Student Succeeds Act and submit reports, including monthly statistics.
