Job Details
This position requires ICU EXP- 2 plus yrs current ICU LTACH experience- BLS, ACLS, NIH; 1 : 2 ratios; Meditech EMR, Must have KY or compact license.
DISCLOSURE : ALL Facilities work a Schedule of 36 / 48 for the two week pay period unless notated otherwise on job order. Ratios can fluctuate with census, call ins, etc. Orientation is bi-weekly. Start date on job is start date needed. Resource must reside 100 miles from facility to be considered. Assignment confirmation needs to be loaded with in 24 hrs. of accepting offer. DO NOT SUBMIT CANDIDATE TO MORE THAN ONE JOB AT A TIME.
Job Requirements
Minimum of 2 years ICU experience required.
Must have KY or compact license.
Schedule Information
36 / 48 schedule for the two week pay period.
Orientation is bi-weekly.
Unit Specific Information
Unit Exam specific to the contractor’s scope of practice within one year prior to start date. Annual update required. Actual document required; completion certificate not accepted.
Additional Details
Proof of 2 MMR vaccinations or a positive titer are required prior to start. If low / equivocal provide proof of first dose to start. 2nd dose required within 28-30 days of first dose.
Positive titer or proof of 3 Hep B vaccine series or declination can be accepted. Annual update required.
Annual Physical or Physician note / statement within 1 year prior to start. Signed by a MD, DO, PA or NP. Must state clear to work; free of restrictions. Annual update required.

Valid 10 years from the administered date. Td will not be accepted. Declinations are acceptable.
If positive TB record; Requires Proof of Chest X-Ray with in last 5 years to be accompanied by TB questionnaire.
