Day Habilitation Program Main Function:
Manage the day-to-day operations of the Faith-Based Day Habilitation program; supervise a team of assistants providing direct service; work independently and use personal and professional discretion to implement new approaches and/or problem solving.
Duties and Responsibilities:
1. Manage Daily Operations:
· Consult with the Executive Director/Community Leader in setting, achieving and evaluating program objectives and goals.
· Know and carry out L’Arche policies and procedures. Develop and revise program policies and procedures and ensure Program Policy and Procedures Manual is up-to-date.
· Oversee daily billing and operations, including transportation issues, in keeping with agency policies and budget guidelines.
· Regularly review status of physical plant(s).
· Schedule assistants for Day Hab. Arrange, coordinate and personally provide coverage when necessary.
· Coordinate with family members/advocates and other Provider Agencies involved with assigned participants.
· Communicate regularly with Community Leader, Service Coordinators and the residential program and relevant clinical staff and participate in staff meetings.
· Learn the software OPWDD uses for case management, client tracking, billing, program planning, and information management (i.e. Foothold)
2. Supervise Assigned Assistants:
· Maintain the integrity of the mission and identity of the faith-based day rehabilitation program as a program with and through the L’Arche Community.
· Orient, train and evaluate assigned assistants in all relevant areas but including individual day habilitation plans, regulations, billing and compliance.
· Conduct regularly scheduled functional accompaniment.
· With Head of House, write initial evaluation period review and annual performance reviews.
· Document performance concerns and follow-up with disciplinary action if necessary.
3. Manage program expenses in keeping with budget guidelines. Assist in the development of an annual budget:
· Understand departmental budget and assist in its development.
· Review financial printouts and work with the Director to modify budget and/or expenses as needed.
· Evaluate the adequacy of funding to provide services as outlined in assigned participants’ Individual Day Hab Plans.
· Understand rate setting system and monthly spending in order to maximize rates/billing. Turn in monthly billing to Business Office and notify Business Office of any status changes which might impact billings and/or collections.
· Ensure that participant rights are protected with regard to their money and possessions.
· Promptly review assistant timesheets, expense reimbursement requests and other administrative paperwork for accuracy and submittal to the Office Manager.
· Provide appropriate documentation for billing to the Office Manager.
4. Develop and Implement Individualized Day Habilitation Plans:
· Coordinate assessments.
· Facilitate semi-annual and annual meetings.
· Listen to participants’ needs and wishes.
· Write and revise individual plans and goals according to OPWDD and L’Arche procedures.
· Maintain files of assigned participants.
· Oversee the official, confidential record developed and preserved on each enrolled participant in compliance with HIPAA regulations.
· Ensure the well-being and safety of participants.
5. Monitor Service Delivery:
· Maintain daily progress notes and monthly summaries.
· Check notes 7 days a week, twice a day.
· Identify untoward incidents; ensures proper incident notification; debriefs assistants; coordinates submission of required documentation. Participates in the agency-wide incident review process.
· Relate activities to the goals identified in participants’ individualized plans.
· Utilize proper forms and procedures for documentation.
· Work with ACCESSCNY to provide quarterly and annual reports.
6. Facilitate regulatory compliance; including the State’s audit/review process:
· Maintain current information regarding applicable regulations.
· Insure that medication, behavioral interventions and emergency safety procedures are developed and practiced. Monitor any medication, behavioral or evacuation drill logs. (Protective oversight).
· Train departmental staff regarding regulatory requirements including OSHA regulations.
· With the Executive Director, meet with and/or accompany state review/audit personnel whenever they visit to review Day Hab.
· Analyze any “Statements of Deficiency” and write required “Plans of Corrective Action” for this service area.
7. Foster Community Inclusion:
· Design activities to maximize community participation and integration of program participants with people who do not have disabilities.
· Research and utilize community resources.
· Support participants to use a variety of modes of transportation, including public transportation where appropriate.
8. Demonstrate Teamwork and Professionalism:
· Attend and participate in Coordinators’ meetings.
· Role model appropriate dress and behavior at work.
· Represent L’Arche positively to other agencies and the larger community.
· Participate in L’Arche work groups, L’Arche events and celebrations; fund raising, strategic planning and assistant formation activities.
9. Manage On-Call Responsibilities:
· Be available to respond to emergency situations as they arise.
Quality Assurance Main Function:
Facilitate an on-going regulatory compliance and quality improvement program.
Duties:
1. Compliance and Quality Improvement Audits
· Conduct regularly scheduled program audits.
· Conduct observations of various services to ensure regulatory compliance and adherence to quality standards.
· Review other systems (i.e. training, incident, personal allowance) as part of the internal audit process.
· Make recommendations to Administrative staff for program specific systemic improvements.
2. OPWDD and OMH Certification
· Assist with writing Plan Of Corrective Action (POCA) in the event of an audit Statement of Deficiencies.
· Monitor implementation of Plan Of Corrective Action as part of internal audit.
3. Training
· Organize applicable regulations and make them accessible to program staff as a resource. Act as a resource to program staff re: applicable regulations.
· Coordinate dissemination of new material to appropriate staff.
· Ensure staff completion of mandatory and annual training in conjunction with AccessCNY training staff and Assistant Director.
· In conjunction with the Assistant Director, develop and implement training modules in response to compliance issues and to identified staff and participant needs.
· Assist agency staff in understanding and effectively using the Foothold electronic documentation system.
4. Community and Teamwork
· Participate in Agency and community meetings, committees and task teams as identified by the Community Leader.
· In coordination with the Executive Director, update and maintain the L’Arche Syracuse website and Facebook page.
5. Other duties as assigned

Pay: $19.00 - $22.00 per hour
