Duties and Responsibilities
- Develop, coordinate and oversee the initial and ongoing assessment activities, initial development and ongoing revision of individual service plan
- Monitoring of implementation of plan
- Coordinate with community agencies to carry out the treatment plan
- Provide any necessary direct services as indicated in treatment plan
- Coordinate service plans with involved family, caregivers, medical, psychiatric and other providers
- Supervise and educate other team members in regards to the provision of quality and accurate services within a group home environment
- Maintain all applicable professional, legal and ethical standards, including confidentiality, dual relations, and informed consent
- Provide documentation, reports, and records to meet State, Federal, Regional and departmental standards and requirements: Clear, organized, timely documentation
- Provide individual therapy, family therapy, and group therapy weekly as assigned
Skills and other functions:
- Work effectively within a multidisciplinary team and provide clinical formulations that clearly direct treatment considerations and focus
- Provides direct clinical supervision to staff as needed or assigned
- Complete other assignments and tasks as directed
- One (1) year of documented experience a licensed or licensed eligible position
- Computer literate
Qualifications:
- Physician or
- Licensed clinical psychologist, or
- Licensed professional counselor, or
- Licensed clinical social worker, or
- Licensed substance abuse treatment practitioner, or
- Certification as a psychiatric clinical nurse specialist, or
- Licensed eligible in either above
- At least one year of clinical experience
Other requirements:
- Successful completion of criminal history background
- Successful completion of central registry check
- Successful completion of CPR and First Aid course
- Successful completion of crisis management course as required'
