Job Summary
We are seeking a dedicated and compassionate Physical Therapists and Physical Therapy Assistants to join our dynamic healthcare team. The ideal candidate will work under the supervision of a licensed Physical Therapist/Physical Therapy Assistant to provide high-quality patient care in various settings, including skilled nursing facilities, houses, and outpatient environments. This role is essential in helping patients achieve their rehabilitation goals and improve their overall physical function.

Responsibilities

• Assist the Physical Therapist in implementing treatment plans and care plans tailored to individual patient needs.
• Conduct therapeutic exercises and activities as prescribed by the Physical Therapist.
• Monitor patient progress and report any changes or concerns to the supervising Physical Therapist.
• Educate patients and their families on treatment procedures, exercises, and home care strategies.
• Maintain accurate documentation of patient treatments, progress notes, and other relevant information.
• Ensure a safe and clean environment for patients by adhering to infection control protocols.
• Collaborate with interdisciplinary teams to optimize patient care outcomes.

Skills

• Knowledge of medical terminology related to physical therapy practices.
• Experience in home settings, skilled nursing facilities, or outpatient clinics is preferred.
• Familiarity with occupational health practices and geriatrics is a plus.
• Strong communication skills for effective interaction with patients, families, and healthcare team members.
• Ability to develop and implement care plans that address the specific needs of patients.
• Proficiency in assessing patient conditions and adapting treatment techniques accordingly.
• Commitment to providing compassionate care while maintaining professionalism at all times.
