Purpose
Children / Adolescent and Adult Services are home-based mental health rehab services provided to consumers between the ages of five (5) and up.
Children / Adolescent and Adult Rehabilitation Specialist (QMHP) is designed to provide face-to-face rehabilitative services to assist assigned individuals in achieving an optimal quality of life including symptom management, medication education/monitoring, community living skills, chemical dependency treatment, employment related skills to increase consumer independence and ability to maintain community tenure (16 years and older) and provide consumer with evidence- based techniques to improve life/coping skills to aid in acclimating to the community.
Rehabilitation Specialist will also provide skills training for parents to assist in coping and become knowledgeable with child's diagnosis, identified behaviors, and know how to advocate for their child.
Rehab services for children and adolescents takes place on weekends and after school; unless the school allows it, services can be provided during school hours. If Rehab specialist must be able to accommodate services after the consumer gets out of school and/or weekends if needed.
Qualifications:
· Education: Bachelor's Degree from an accredited college or university with a major in social, behavioral or human services related field (Required) (Psychology, Social Work, Medicine, Nursing, Rehabilitation, Counseling, Sociology, Gerontology, Special Education, Physician Assistant, Educational Psychology, Human Growth and Development, Early Childhood Education, Early Childhood Intervention
· Valid Driver's license with safe driving record and proof of automobile insurance (Required)
· One (1) year paid direct care experience in a community setting with knowledge of MH rehabilitative techniques and community resources. (Preferred)
· Bilingual (English/Spanish) (Preferred)
· Adults Needs and Strengths Assessment Certification (ANSA)
· Child and Adolescent Needs and Strengths (CANS)
· Bilingual (English/Spanish)
Essential Duties and Responsibilities:
· Work with consumers from age five (5) and up.
· Provides direct care services to consumer as directed
· Ensures all consumer have current assessments and treatment plans as per regulatory requirement
· Manage rehabilitation caseloads in accordance to performance requirements
· Ensure all services delivered according to prior authorization
· All documentation to be completed within one (1) business day of services provided
· Ensures regulatory quality of all documentation per unit
· Ensures compliance with Medicaid and/or Third-Party Payor regulations
· Attend training/meetings as directed by Director or Team Leader
· Be willing to deliver medications as needed.
· Be available to team during hours of operations and/or as Supervisor/Team leader require keeping in mind that children must be seen afterschool and weekends in order to meet the needs of the caseload
· Ensures work behavior is compliant with safety regulations
· Provides clinical services in accordance with approved curriculum and treatment plan
· Implements Rehabilitative Plan within five (5) days of each new authorization and within twenty-four (24) hours of consumer's discharge from an inpatient facility
· Provides rehabilitative services including general symptom management, medication education/monitoring, community living skills, chemical dependency treatment (COPSD), and employment related skills to increase consumer' independence and ability to maintain community tenure
· Ability to work with parents providing skills training and guidance
· Develops and maintains cooperative working relationship with community agencies
· Maintains flexibility to deal with clinical emergencies
· Ensures that each consumer on given caseload is seen as prescribed in service plans
· Documents attempt to provide services and any no-shows; client no show rate must not exceed 25%
· Maintains open communication with the court system and attends required scheduled court hearings
· Transports or arranges for consumers to attend appointments as needed Ensures that children are transported safely according to proper child restraint seats (car seat, booster seats according to weight specification)
· Provide community crisis resolution
· Participates in identified assigned function in compliance with and adherence to required federal/state/corporate mandates, regulation, policies, and procedure
· Maintains open communication with assigned team leader
· Certification must be maintained at all time: Child and Adolescent Needs and Strengths (CANS) and Adult Needs and Strengths Assessment (ANSA)
· Requires the desire to learn to manage ARD meetings for children, Special education rules and be an advocate for the child in the school setting
· Strong organizational skills to manage caseload and include trainings on subjects pertinent to working with children and parents as identified
