Description
POSITION OVERVIEW :
The Lead Clinician will be providing mental health support to the Transitional Age Youth 18-25 years of age, involved in gangs, groups, and gun behavior within Alameda County who are enrolled in the TAY Restorative Reentry Program.The Lead Clinician will provide direction to and be instrumental in providing short-term mental health support to clients. Supporting enrolled clients who need urgent assessment and assistance with issues related to mental health crises or community needs. The Lead Clinician will work closely with the Supervisor and Case Managers to best serve clients enrolled in the Restorative Reentry Program. The clinician should investigate presenting issues and subsequently issue treatment. The Lead Clinician should ensure to track patients' responses to prescribed treatment plans.

Essential Job Functions and Responsibilities :
- Reviewing patients' health-related presentations.
- Providing effective but resource-sensitive treatment.
- Work with Restorative Reentry Staff to de-escalate situations, including creating a peaceful, private environment so that the clients may stabilize.
- Co-create a plan for the future with the client, including identifying the next steps and relevant social service resources.
- Provide culturally responsive support during all in-person / contact and virtual visits.
- Utilize Motivational Interviewing and other evidence-based interventions throughout.
- Provide follow-up to the client as appropriate, to ensure that service connections are made and that the plan elements are moving forward.
- Connect participants and provide "warm hand-offs" to other community-based providers, to the greatest extent possible, including following up on both internal and external referrals.
- Observing appropriate mental health care provision guidelines.
- Assumes responsibility for doing assigned work and for meeting deadlines. Completes assigned work on or before deadlines in accordance with directives, policies, standards, and prescribed procedures.
- Demonstrates an awareness of and sensitivity to clients including cultural and ethical beliefs; and implements care in a thorough, skillful, consistent, and continuous manner.
- Knowledge of community resources for client referrals.
- Attends work regularly and adheres to policies and procedures regarding absences and tardiness. Provides adequate notice to supervisor and management with respect to vacation time and time-off requests.
- Demonstrates knowledge of legal issues including client confidentiality and risk management in all aspects of client care and department functioning.
