Description

POSITION OVERVIEW:

The Lead Peer Case Manager will support the Program Supervisor in providing coordination of program staff and activities as assigned by the Program Director. The lead peer case manager will help participants navigate their roles within their Family units,  strengthen their parenting and/or communication skills, and foster healthy relationships with their children and/or Families. The Lead Peer Case Manager will support the program supervisor by performing tasks and functions assigned to the supervisor in their absence, and also represent La Familia in county meetings and meetings with partners, employers etc. The role also includes attending appointments with clients, facilitating communication between the Participant’s Supervising agency and helping their Supervising agency better understand the Participant’s circumstances and challenges; helping Participants fill out forms; serving as a model or example of what is possible; and otherwise, being a safe, supportive resource and advocate for the Participants. This role will uphold and champion the agency’s mission, vision, and values and contribute to a collaborative and inclusive work culture.

The hours for this position are: Monday thru Friday 9:00am to 6:00pm, with services available in the evenings and on weekends by appointment.

Essential Job Functions and Responsibilities:

  • Manage caseloads for clients to provide and track case management and care coordination services, including:
    • Develop and maintain rapport with participants and maintain rapport and trust through consistent, regular meetings, including proactively communicating and soliciting updates from participants and consistent use of Motivational Interviewing techniques.
  • Support Program Supervisor by performing tasks and functions related to various administrative and managerial duties, including, but not limited to: 
    • Perform administrative duties assigned to Program Supervisor in absence of Program Supervisor; 
    • Provide feedback to and facilitate communication to Program Supervisor from and about staff and program needs and morale; assist FRP staff to troubleshoot and problem-solve client and program operational needs and concerns; 
  • Must be able to serve as peer mentors or credible messengers, who can establish a deeper level of trust with Participants. 
  • Must help clients stay engaged and motivated in making progress towards the goals that are articulated. 
  • Must provide resources and support services that will remove barriers to goals.
  • Must track progress towards goals. 
  • Must be able to support participants as they navigate through their reentry journey. 
  • Facilitate the planning process of an individual's case plan; oversee, evaluate and monitor the implementation of case plan objectives.
  • Provide outreach engagement, linkage, and follow up services to clients.
  • Act as a role model, with the intention of inspiring, engaging, and advocating for client’s wellness and rights.
  • Research and provide referral resources for participants in order to help them meet their needs.

Client Connection and Assessments:

  • Peer must connect to clients as early as possible in their re-entry process.
  • Must reach out to probation and jails to request access  to creating relationships with individuals who are incarcerated.
  • Increase awareness of Family Reunification services among those who are incarcerated. 
  • Share ways to help strengthen Familial bonds and start the reunification process prior to release. 
  • Must serve as their support, advocate, translator, champion, cheerleader, and peer-to-peer mentor.
  • Be able to meet with county and respective partners when meetings are held. 

Contract Management:

  • Serve as a liaison with all funders on all programmatic and operations issues.
  • Attend all funder-required meetings, events, and training.
  • Ensure delivery of program elements in compliance with contract language.
  • Submit all necessary documents and reports to funders by deadlines.
  • Work with the supervisor and La Familia Administration to ensure appropriate contract expenditures. 

Other 

  • Attend trainings and participate in events relevant to key responsibilities
  • Track and regularly report on key metrics for responsible function areas
  • Leads and collaborate on special projects related to agency objectives as appropriate 

Qualifications


QUALIFICATIONS & EXPERIENCE:

  • Bachelors’ Degree in Community Development, Sociology, Criminal Justice or any other related field.
  • At least 2 years of experience in case management, coaching and peer mentoring working with system impacted or justice-involved individuals. 
  • Demonstrated experience working in programs that work with low-income and other vulnerable communities.
  • Demonstrated capacity and commitment to work in cooperation with a diverse team of professionals in regards to race; class; gender; sexuality; disability; age; immigration/citizenship status; religion/spirituality; and other social statuses.
  • Demonstrated experience working with youth and adults of diverse cultural backgrounds, knowledge of and respect for a variety of cultural communities in Alameda County preferred.
  • Spanish/English bilingual preferred but not required.  
  • Ability to work comfortably with various stakeholders, including but not limited to Superior Court, District  Attorney’s Office, Office of Administration, Public Defenders Office, Probation Department, and  funders.
  • Non-Profit and Community Based Organization experience highly desirable.
  • Demonstrates and models excellent “customer service” orientation, especially in the ability to interact with people at all levels within and outside the agency. 
  • Ability to use appropriate interpersonal skills, including treating others with courtesy, sensitivity, and respect, and considering and responding appropriately to the needs and feelings of different people in different situations.
  • Thinks independently and can make informed decisions when needed. 
  • Exercises extreme discretion with the ability to hold, balance and transmit public and confidential information needed by various parties. 
  • Proficient using MS Office products, particularly Word, Excel, Outlook and PowerPoint.
  • Acquires or maintains proficiency in using Google Drive products, including Docs, Sheets, and Forms.
  • Valid CA Driver’s license
    • Auto insurance:  Personal vehicles may be used for business related purposes; it is recommended you inform your auto insurance carrier.
