What You’ll Do

The Case Manager provides case management and coordination of care services to clients enrolled in the Estrella de Vida transitional living and day program.

Essential Responsibilities

  • Serve as a point of contact and ensure ongoing collaboration with the clinical team and other involved parties as appropriate. Collaboration may include a person’s health plan, referral source, family, behavioral and general medical and dental health care providers, community resources, and other involved supports including educational, social, judicial, community, and other State agencies. Involve natural supports as preferred by the client.
  • Facilitate, schedule, and provide follow-up Adult Recovery Team Meetings. .
  • Work collaboratively with the clinical team to ensure that all services prescribed in the individualized service plan are implemented.
  • Make referrals to collaborating partners and other community services, as the client's needs and service plan indicate.
  • Conduct coordination of care activities related to continuity of care between levels of care and across multiple providers, services, and supports
  • Provide direct support, as needed, in order to support the client in maintaining stability in their placement and in developing skills as outlined in their individualized service plan.
  • Provide coverage to the program as assigned, possibly including admissions, facilitating groups, meal service, and recovery technician duties.
  • Deliver at least 40 hours of work per week, with a goal of 30 hours of direct client contact. Productivity standards are established at a minimum of once a year, and as needed, due to contract changes.
  • Maintain all client records in current status and in accordance with agency policies and procedures, as well as health plan, state, and federal requirements. Produce complete, timely, thorough, quality clinical documentation of all services rendered, both billable and non-billable, as required.
  • Maintain the person’s comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, discharge planning).
  • Provide transportation to the consumer as appropriate and determined by the housing team
  • Attend regular scheduled weekly supervision meetings. Seek out additional supervision as needed
  • Participate in all regularly scheduled administrative, in-service, and department staff meetings and trainings.
  • Recommend to the Clinical Coordinator or Department Manager changes that would improve service delivery. Implement changes and new programming as directed.
  • Evening and weekend hours may be required to accommodate the schedules of children, families, and adults.
  • Adhere to the agency's policies and procedures. Comply with the agency's standard of care for service delivery. Assist with Quality Management, Quality Assurance, and Utilization Review.

What You’ll Provide

  • Bachelor’s degree in Psychology, Social Work, Family Studies, or other behavioral health field OR 4 years of experience in direct behavioral health services to meet BHT requirements.
  • Excellent oral and written communication skills
  • Detail-oriented and strong organizational skills
  • Strong internal and external customer service skills.
  • Demonstrated ability to remain calm in crisis situations, work independently and cooperatively, recognize personal limitations, and relate positively and with empathy to all persons regardless of age, race, creed, gender or sexual orientation.
  • Maintain professional conduct consistent with ethical professional practice and the policies and procedures of the agency.
  • Maintain a professional demeanor and positive working relationships with internal and external team members, stakeholders, and community and business partners.
  • If driving while on agency time or on agency business, a valid Arizona driver's license and appropriate liability insurance, and approval for liability coverage with EMPACT - SPC’s liability insurance carrier is required. At time of hire, a 3-year state driving record with no major infractions or excessive driving violations/tickets is required.
  • Staff members who provide direct client services must be at least 21 years old. Staff members who provide support services (administrative or clinical) must be at least 18 years old. All employees will have a minimum of a high school diploma or equivalent.
  • The typical work environment will be working indoors and outdoors
  • Employees must be able to lift up to 50 pounds at times.
  • While performing the duties of this job, the employee may be regularly required to stand, sit, talk, hear, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard.
  • Occasional travel to training and out-of-state locations may occur as-needed.
