">Job Summary:
">The Case Manager assumes primary responsibility for case management of the assigned caseload, including but not limited to assessment, treatment planning, individual and group counseling, charting, discharge and continuing care planning, liaison activity with significant others, communication as necessary with referring agents and third party payers, and facilitating didactic groups as assigned.
">Key Responsibilities:
">- Assess and develop treatment plans for assigned clients.
- Provide individual and group counseling sessions as needed.
- Chart client progress and maintain accurate records.
- Coordinate discharge and continuing care planning for clients.
- Communicate effectively with referring agents and third party payers.
- Facilitate didactic groups as assigned.
Requirements:
">- Licensed as an LCDC, LPC, LCSW or other Qualified Credentialed Counselor in Texas.
- Prior experience working with chemically dependent patients.
- Bachelor's or master's degree in a Human Services field preferred.
- Basic computer skills required.
