">We are seeking a qualified Case Manager to join our team. The ideal candidate will have prior experience working with chemically dependent patients and be licensed as an LCDC, LPC, LCSW or other Qualified Credentialed Counselor in Texas.
">Responsibilities:
">- Assess and develop treatment plans for assigned clients.
- Provide individual and group counseling sessions as needed.
- Chart client progress and maintain accurate records.
- Coordinate discharge and continuing care planning for clients.
- Communicate effectively with referring agents and third party payers.
- Facilitate didactic groups as assigned.
Requirements:
">- Licensed as an LCDC, LPC, LCSW or other Qualified Credentialed Counselor in Texas.
- Prior experience working with chemically dependent patients.
- Bachelor's or master's degree in a Human Services field preferred.
- Basic computer skills required.
