">Job Description:
">The Case Manager will assume primary responsibility for case management of the assigned caseload, including assessment, treatment planning, individual and group counseling, charting, discharge and continuing care planning, liaison activity with significant others, communication as necessary with referring agents and third party payers, and facilitating didactic groups as assigned.
">Qualifications:
">- Licensed as an LCDC, LPC, LCSW or other Qualified Credentialed Counselor in Texas.
- Prior experience working with chemically dependent patients.
- Bachelor's or master's degree in a Human Services field preferred.
- Basic computer skills required.
