POSITION SUMMARY
The position provides comprehensive, patient-centered care across various clinical settings, ensuring the highest standard of medical service for our community. The Nurse Practitioner will assess, diagnose, treat, and manage both acute and chronic health conditions, working closely with physicians, specialists, and other healthcare providers to deliver quality care. CORE FUNCTIONS
1. Conduct thorough patient assessments, including taking medical histories, performing physical exams, and ordering and interpreting diagnostic tests.
2. Diagnose and manage a wide range of health conditions, both acute and chronic, and prescribe medications as appropriate.
3. Provide patient education on disease prevention, health promotion, and the management of medical conditions.
4. Collaborate with physicians, specialists, and interdisciplinary teams to develop and implement individualized care plans.
5. Monitor patient progress, adjust treatment plans as necessary, and ensure appropriate follow-up care.
6. Document patient interactions, assessments, and treatments in compliance with hospital policies and state regulations.
7. Participate in patient care coordination, including referrals to specialists and follow-up appointments.
8. Engage in continuous professional development, staying current on advancements in healthcare, clinical best practices, and healthcare regulations.
9. Participate in quality improvement initiatives to enhance patient care and contribute to the overall mission of La Paz Regional Hospital and Clinics.
10. Provide guidance and mentorship to other healthcare staff as needed.
MINIMUM QUALIFICATIONS
Masters Degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
Current valid Registered Nurse/Nurse Practitioner License in the state of Arizona or eligible to obtain is required. BLS, ALCS, and PALS is required. American Academy of Nurse Practitioners Certification and Controlled Substance Registration Certificate (D.E.A) is required.
Must have CDL Certification within 6 months of hire date to do physicals.
Strong Communication, interpersonal, and Collaborative skills. Ability to manage a high caseload while maintain patient safety and quality care.
Practitioner is expected to see patients scheduled approximately every 15 minutes; new patients every 20-30 minutes and be able to adapt to changes in the schedule as needed for patient care. It may also be necessary for the Practitioner to make additions to the schedule for Urgent Care type patients. Time will be set aside at the end of the day to follow up on consults, ordered lab, and refill requests, etc.
PREFERRED QUALIFICATIONS
Previous experience in a Nurse Practitioner role, particularly in rural health setting.
Familiarity with electronic health record (EHR) systems, specifically Meditech.
Additional related education and/or experience preferred.
