We are currently seeking an exceptional Director of Care Management to join our dedicated team.
Position Overview :
As the Director of Care Management, you will lead a team of Registered Nurses, Caregivers, and Social Service staff to enhance well-being and health management of campus residents and Adult Day Services participants. You will collaborate with these teams on assessments, planning, facilitation, and advocacy to meet each individual’s comprehensive health needs. You will play a key role in program development, performance improvement, and coordinating residents’ progress, ensuring that all efforts align with La Posada’s strategic goals. Additionally, you will provide direct services to residents and conduct off-campus assessments as needed.

This person must have the following skills :
Skills and Abilities
Education : Bachelor or Associates Degree in Nursing from accredited institution or is an advanced degree nurse practitioner.
Experience : Minimum of 5 years of clinical experience, with 2 years of administrative, supervisory, or charge nurse experience
Computer Skills : Proficient computer skills.
Certifications & Licenses : Registered Nurse with a current license in Arizona.
Other Requirements : Must be familiar with Private Duty guidelines for Home Health.
