Job Summary:

The Family Nurse Practitioner is a licensed independent practitioner who provides routine patient health care, women’s health and management of patients with acute and chronic complaints and conditions to a wide variety of ages in a family practice setting seeing on average three to four patients an hour.

Essential Job Responsibilities:

Listed are a representative summary of the major duties and responsibilities of a Nurse Practitioner at La Red Health Center. This is not a comprehensive or exhaustive list or all job functions.

  • Obtains and documents complex health history, performs physical examinations with assessment of normal or abnormal findings, educates patients on preventative health measures.
  • Provides patient care based on practice guidelines standards of care and federal/state laws and regulations.
  • Documents and maintains patient’s records of care on Health Center EMR.
  • Perform complex diagnostic and therapeutic procedures, orders laboratory, x-ray, EKG’s and other special exams in diagnosing medical and surgical problems and prescribing necessary treatment and services for quality patient care.
  • Develops and institutes appropriate treatment plans per protocol. Treatment will include the care of acute and chronic medical problems as well as contraceptive services.
  • Educates and counsels patients about diet, exercise, health status, use of drugs, expected effects of treatment, child care, birth control, family planning, safe sex. and other therapeutic health maintenance matters.
  • Consults with CMO and other health providers, and community resources to prevent/resolve patient problems or concerns. Makes referrals as patient’s needs require.
  • Documents patient assessment, counseling, interventions and treatment plans etc. by using established Health Center EMR documentation practices. Monitors the effectiveness of interventions.
  • Orders and furnishes medications according to written standards.
  • Participates in multidisciplinary teams to improve patient care processes and outcomes.
  • Participates in continuing education courses, staff meetings and clinical meetings as assigned.
  • Maintain productivity satisfactory and consistent with agency’s goals.
Requirements:

Education and/or Experience:

Masters Degree Nurse with advanced practice skills as a Nurse Practitioner.

Candidate must be a Delaware Licensed Nurse Practitioner

One to two years experience working with the underserved, low-income or homeless population.

Ability to work with diverse multi-cultural patients and staff.

Language Skills:

English

Bilingual skills (English/Spanish) a plus.

Performance Requirements:

Knowledge:

  • Knowledge of nursing and family practice care processes; health care systems, structure and functions; community resources, hospitals.
  • Knowledge of wellness, illness, life stages, growth and development, human behavior, psychosocial factors and alternative health care treatments that are ethnically predominant.
  • Knowledge of medical record documentation requirements, and federal/state laws regarding release of medical records or health care information

Skills:

  • Skill in time management, problem solving, multitasking, prioritizing and medical care coordination.
  • Skill in initiating appropriate crisis interventions and emergency response
  • Skills in patient triage in person and on the phone.
  • Skill in dealing with large quantities of information in an organized manner.
  • Skill in using computer and medical records software.
  • Skill in posting all pertinent information, laboratory and test results information into EMR, meeting Center standards for timeliness.

Abilities:

  • Ability to analyze options and counsel patients and families about care and make referrals to other providers and resources.
  • Ability to engage patients and families in the development and implementation of a care plan.
  • Ability to read and interpret physician orders and notes from other providers.
  • Ability to use excellent interpersonal skills in building rapport, trust, and patient centered care.
  • Ability to analyze data, promptly review and post laboratory and test result.
  • Ability to analyze medical records for completeness and accuracy, paying attention to detail.
  • Ability to manage time and appointment scheduling effectively.

Equipment Operated:

Wide range of standard medical equipment including portable Ultrasound equipment, syringes, speculum, oxygen tanks, wheelchairs, thermometers, etc. Computer use and proficiency required.

Work Environment:

Medical exam/treatment rooms and medical offices in Health Center. Controlled lighting, air quality, and temperature settings. Environmental hazards may be unpredictable including exposure to communicable diseases and biohazards.

Mental/Physical Requirements:

Involves standing, walking, kneeling, bending, grasping, manipulating and squatting and lifting (up to 50 lbs.). Walking or standing may be more than four (4) hours per day. Must be able to work in a standard medical office setting including use of a computer, and standard office equipment. Must have ability to read computer screens, printed materials and hearing and speech to communicate in person as well as over the phone. Must be able to attend meetings or make presentations at various sites when required. Stress can be triggered by an individual’s response to workload or specific cases.
