Position Description
Position: Nurse Practitioner specializing in Primary Care and Chronic Disease
Management.
Location: Lac Courte Oreilles Community Health Center
Hours: Monday to Friday, 8:00 AM to 4:30 PM (with the option of working four
10-hour shifts)
Salary Range: Negotiable

Description of Position:
We are seeking a dedicated and compassionate Nurse Practitioner (NP) specializing in Primary
Care and Chronic Disease Management. The ideal candidate will play a key role in providing
high-quality, patient-centered care to individuals with both acute and chronic health conditions.
The NP will work closely with a multidisciplinary team to deliver comprehensive primary care
services, focusing on managing long-term conditions such as diabetes, hypertension, asthma,
heart disease, and more.
Qualifications:
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an
accredited program.
Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse
Practitioner (AGNP).
Valid Nurse Practitioner license in the state of WI.
Certification through a recognized body, such as the American Association of Nurse
Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
Experience in primary care or chronic disease management preferred.
Strong clinical knowledge of chronic conditions such as diabetes, hypertension, asthma,
cardiovascular diseases, and respiratory disorders.
Strong clinical skills and the ability to perform physical exams and diagnose medical
conditions.
Excellent communication skills to interact effectively with patients, families, and
healthcare teams.
Ability to work independently as well as part of a multidisciplinary team.
Knowledge of current medical practices, treatments, and guidelines.
Current DEA and DPS Certificate of Registration required.
Medicaid and Medicare provider eligible required.
Proficiency in electronic health records (EHR) and other clinical documentation tools.
Valid WI Driver’s License
Valid vehicle insurance
Be able to pass a pre-employment drug test and random drug screens
Be able to pass a background check
Ability to maintain confidentiality and observe all requirements of the Federal Privacy
Act and HIPAA regulations.
Major Duties and Responsibilities:
Conduct comprehensive assessments, physical exams, and medical histories for patients
of all ages.
Diagnose and treat acute and chronic conditions, with a focus on chronic disease
management (e.g., diabetes, hypertension, cardiovascular diseases, COPD, etc.).
Order and interpret diagnostic tests (e.g., lab work, imaging).
Prescribe medications, treatments, and therapies as needed.
Provide patient education on managing chronic conditions and promoting overall health
and wellness.
Provide routine check-ups, screenings, and vaccinations.
Educate patients on self-management strategies, lifestyle modifications, medication
adherence for chronic disease management, and preventive measures to maintain health
and prevent disease.
Work in collaboration with physicians, other NPs, nurses, and healthcare professionals to
create and implement patient care plans.
Refer patients to specialists when necessary and follow up on their treatment plans.
Maintain accurate and up-to-date patient records in compliance with healthcare
regulations.
Document all interactions, assessments, and care provided in electronic health records
(EHR).
Advocate for patients' needs, ensuring they receive the appropriate care and resources.
Address patient concerns and help navigate the healthcare system.
Empower patients to take an active role in their health by providing education on their
conditions and treatment options.
Develop and implement individualized care plans to manage and prevent chronic
conditions, standards of care, and professional practice guidelines.
Recommend interventions to modify behavior associated with health risks.
Work with the Pharm D on Medication Reconciliation/synchronization.
Utilize the Patient Centered Medical Home model approach to achieve primary care
excellence that best suit’s the patient’s needs.
Conducts physical and psychological assessments to evaluate the total health care needs
of the patient.
Provide emergency care as needed or required, including lifesaving procedures to
stabilize patients for transport to hospital.
Complete sufficient, timely, and accurate documentation in the Electronic Health Record
within 2 days of date of patient visit or patient contact or patient care activity. Must be
completed within 48 hours for best patient care and revenue cycle processing.
Reporting of Adverse Incidents immediately to supervisor and Health Director for timely
resolve.
Participation in Emergency Preparedness and Incident Command as assigned.
Perform other duties as assigned by the Medical Director or Health Director within scope
of practice.
Supervision and Guidance:
The Nurse Practitioner will work under the direct supervision of the Medical Director with
oversight by the Health Director.
