- Assess residents' physical conditions and develop individualized treatment plans
- Provide therapy sessions focused on improving mobility, strength, and function
- Collaborate with the interdisciplinary care team to ensure holistic resident care
- Educate residents and families on exercises and recovery plans
- Maintain accurate and timely documentation What We're Looking For :
- Active Physical Therapist (PT) license
- Strong clinical skills and a passion for resident-centered care
- Excellent communication and teamwork abilities
