Caseworker

Job Description Summary

Responsible for visiting clients, new or established, and conducting the Louisiana Independent Living Assessments (LILA). Caseworkers will be assigned exclusively to activities related to elderly clientele.

Essential Functions

• Serves as a resource person and collaborates with all agency personnel.
• Conducts assessments on elderly clients (LILA and NFCSP) as assigned.
• Develops a care plan for assisting eligible, elderly clients.
• Communicates with other Agency staff on behalf of clients.
• Visits all clients referred by the Agency and assures that individual appointments are kept.
• Keeps home visitation records on all clients.
• Participates in program evaluations.
• Establishes and maintains cooperative relations with appropriate community service agencies and facilities.
• Maintains good work and public relations-this includes demonstrating friendliness, warmth, and cooperation to participants, co-workers, and supervisors.
• Performs other duties as assigned by the Program Manager.

Position Type and Expected Hours of Work

This is a full-time position. Days and hours of work are Monday through Thursday, 8:00 a.m. to 4:30 p.m. and Friday, 8:00a.m. to 2p.m. Schedule may be adjusted according to agency needs.

Travel

Travel is required for this position. Mileage will be reimbursed at rate being implemented by the agency.

Required Education, Experience, and Requirements

• High school diploma
• Two years of related work experience
• Detail-oriented and able to prioritize
• Must possess clearance of a criminal background check
• Must possess clearance of a drug screen
• Current driver’s license, and valid auto insurance (minimum limits as required by law)
• CPR/First Aid certification

Job Type: Full-time

Pay: $12.00 - $12.01 per hour
