Home Care Coordinator – ODP Waiver Program

Lagom Care is a leading Hispanic Home Care Agency in Pennsylvania, dedicated to providing top-quality waiver services to Medicaid recipients. We are looking for a dedicated individual who looking to work with our intellectual disabilities group. This person will be the case manager at Lagom Care and will head all efforts of the ODP Department.
Position Overview:
The individualis responsible for managing the compliance, scheduling, and clinical aspects of the program, ensuring that all direct support staff and individuals receive the highest quality of care in accordance with state regulations. This position requires fluency in English and Spanish and prior experience with the ODP Waiver program.
Key Responsibilities:1. Compliance Oversight:
- Ensure adherence to all ODP regulations and Medicaid waiver requirements.
- Monitor staff compliance with required trainings, documentation, and certifications.
- Conduct audits of client records, staff files, and service logs to maintain compliance.
- Keep updated with policy changes from the Office of Developmental Programs (ODP) and implement necessary adjustments.
2. Scheduling & Staff Coordination:
- Oversee and manage staff scheduling to ensure proper coverage for all individuals receiving services.
- Ensure that all scheduled shifts comply with ODP requirements and that staff meet all qualifications.
- Address and resolve scheduling conflicts, last-minute call-offs, and service gaps.
- Maintain clear communication with caregivers, clients, and family members regarding schedules.
3. Clinical Support & Client Visits:
- Conduct regular client visits to assess service quality and client satisfaction.
- Monitor client progress and ensure all care plans are followed correctly.
- Act as a liaison between clients, families, and support staff to address concerns.
- Provide feedback and support to direct care staff for improved service delivery.
Qualifications & Requirements:
- Bilingual (English/Spanish) – Required.
- Experience with the ODP Waiver Program – Required.
- Minimum of 2 years of experience in home care coordination, waiver programs, or a similar role.
- Strong knowledge of Medicaid waiver services, ODP regulations, and compliance standards.
- Experience managing staff scheduling, compliance audits, and client care coordination.
- Excellent communication, organizational, and problem-solving skills.
- Ability to travel within the service area to conduct client visits (gas reimbursement provided).
