What are the responsibilities and job description for the School Social Worker (1.0 FTE) - Preschool through Grade 6 position at LaGrange School District 105?
- Attends to crisis situations (e.g. child abuse, acute, chronic, and severe behavior problems, substance abuse, suicide, death, attendance, runaway, etc.).
- Maintains and respects confidentiality of students and staff.
- Work collaboratively with the special education team and general education staff to ensure individual student's needs are met.
- Completes the social developmental study for a case study evaluation (i.e. child interview, family interview, adaptive behavior assessment, classroom teach consultation and observation, review of permanent and temporary file, functional assessment, behavior intervention plan.
- Engage in the problem-solving process with teachers, specialists, and parents to discuss individual student needs and interventions.
- Serve as a leader in the school positive behavior system.
- Evaluates and participates in determining student eligibility for special education placement and school social work services.
- Participates in 3-5-year-old preschool screenings.
- Supports Tier 1 social emotional learning in classrooms.
- Delivers direct and or consultative social work services as indicated on the IEP.
- Delivers direct and or consultative social work services as indicated on the 504 plan.
- Organizes and analyzes social emotional assessment screener for general education K-6.
- Facilitates internal data days with staff analyzing student supports.
- Plan, coordinate, and implement mandated social emotional programs including Erin’s Law, bullying prevention and suicide prevention.
- Works with and communicates regularly with parents as needed during school hours.
- Monitors and provides for behavioral interventions for all students in grades preschool - 6th.
- Assists teachers with classroom behavior management.
- Assist with mental health concerns, behavioral concerns, positive behavioral support, academic, and classroom support, consultation with teachers, parents, and administrators as well as provide individual and group counseling/therapy.
- Organizes community outreach and maintains community relations to support student/family services.
- Completes all required documentation, including, case notes, billing for Medicaid eligible students, and related service logs.
- Participate in staff development and school improvement activities in order to maintain professional growth.
- Perform other related duties as assigned.
