Shift: 8:00 AM - 4:30 PM
Salary: Up to $28 per hour (depending on experience)
Job Description:
Bedrock Rehabilitation and Nursing Center at Lake City is seeking a dedicated Social Services Director to oversee and implement social services programs in our skilled nursing facility. This role involves discharge planning, behavioral health services, care coordination, financial assistance guidance, and community resource referrals to ensure the well-being of our residents. The Social Services Director will work closely with the interdisciplinary team (IDT), residents, families, and community agencies to support each resident’s highest level of physical, mental, and psychosocial health.
Key Responsibilities:
- Coordinate and implement social services programs under the direction of the Administrator and Director of Social Services.
- Assist with discharge planning, Medicaid applications, financial assistance, and referrals to outside agencies.
- Ensure behavioral health needs are met according to the care plan.
- Participate in resident group meetings and provide guidance on resident rights, living wills, and advance directives.
- Stay current on state and federal regulations and recommend policy updates as needed.
- Assist residents with limited English proficiency (LEP) by coordinating translation services.
- Maintain a resource library for social services standards and compliance.
- Collaborate with nursing, therapy, admissions, and administration teams to enhance resident care.
- Participate in facility surveys, assessments, and inspections by regulatory agencies.
Requirements:
- Bachelor’s degree in Social Work, Sociology, Psychology, Gerontology, Special Education, or a related human services field.
- Minimum of 1 year of supervised social work experience in a healthcare or skilled nursing setting working directly with residents.
