What are the responsibilities and job description for the Health Educator position at Lake County General Health District?
Position Summary:
The Health Educator provides education and training, plans, implements, and evaluates activities that promote policy, system, and environmental changes designed to positively impact those who live and work in Geauga County.
Essential Duties:
· Uses data to assess the community’s present and projected health needs by proposing and planning specific health activities to address priorities identified in the Community Health Improvement Plan, Strategic Plan, or other grant deliverable;
· Engages target populations to design methods and activities to be used, timelines and an evaluation process;
· Provides education and training to a wide variety of audiences;
· Communicates and collaborates with community members, key stakeholders, and other Geauga County organizations to address the social determinants of health and achieve health equity;
· Represents Geauga Public Health on community coalitions and at public events;
· Participates in continuous quality improvement activities.
Other Duties & Responsibilities:
· Maintains compliance with grant guidelines, if applicable;
· Prepares and submits to supervisor a monthly report of program activities and progress;
· Maintains a system of record keeping for files, materials and purchases;
· Attends grant meetings and maintains knowledge in the field;
· Receives and screens community requests for information and assistance;
· All health district employees have an emergency response role and may be expected to respond to threats to the public’s health;
· Performs other duties as assigned consistent with position.
Minimum Qualifications:
· Bachelor’s degree in Public Health, Health Promotion, Health Education, or closely related field, or 4-5 years relevant work experience in public health;
· One to two years related experience and/or training; or equivalent combination of education and experience;
· Must have occasional evening and weekend availability to participate in community events. Weekly schedule may fluctuate based on required grant activities.
· CHES certification preferred;
· Must have and maintain a valid State of Ohio driver’s license with an acceptable driving record.
This position will be located at Geauga Public Health and work activities will occur in Geauga County; however, the employing entity is Lake County General Health District.
