What are the responsibilities and job description for the Continuum of Care Program Specialist position at Lake County Illinois?
General Description
This position involves significant responsibility in facilitating coalition meetings, managing and improving data processes, and coordinating multiple improvement initiatives for the Lake County Coalition for the Homeless meetings. This role requires both independent work and collaboration with internal and external stakeholders to improve system coordination and housing access for vulnerable populations. Day-to-day tasks include managing By-Name List data quality to facilitate meetings, training staff, and overseeing the implementation of new programs.
Essential Functions
- Track and input referral data into HMIS.
- Manage and provide regular data reports to Coalition Board members, ensuring high data quality standards.
- Independently prepare for and facilitate committee meetings, task forces, and work groups, including the creation of meeting materials and agendas.
- Conduct training sessions on existing systems and new processes, ensuring coalition members stay up to date on all CoC policies, especially those pertaining to Coordinated Entry.
- Oversee and maintain programs like The Finish Line Funds, Moving Up, new and ongoing voucher programs, coordinating with external partners to maintain these programs.
- Supporting grant writing for the NOFO and other flexible funding opportunities.
- Supporting in the planning and execution of the Point in Time Count
- Identify gaps and barriers to housing and services.
- Onboard various stakeholders, including lived experience members and case managers.
Knowledge Skills Abilities and Education Required
- Strong organizational skills with attention to detail.
- Excellent communication and interpersonal skills, particularly with diverse populations and those in crisis.
- Proficiency in Microsoft Office, Excel and housing databases.
- Knowledge of fair housing laws, affordable housing regulations, and local rental markets.
- Familiarity with Housing Choice Voucher programs, Rapid Re-Housing, and Permanent Supportive Housing and public housing systems is a plus.
Educational Requirements
- Bachelor’s degree in related field or 3-5 years of experience in homeless coalition involvement or working with homeless populations preferred.
Preferred Experience
- Working knowledge of barriers to housing.
- Experience leading and facilitating meetings.
- Experience creating and implementing new projects.
- Experience managing data and creating or presenting reports.
