What are the responsibilities and job description for the Counselor II - Assertive Community Treatment position at Lake County?
The Counselor II is a valuable member of our (ACT) Assertive Community Treatment team who is responsible for evaluating and assessing clients. Plans and implements a treatment plan with clients on assigned caseloads. Conducts case management, individual, group, and family counseling, outreach, education, monitoring, referral, linkage, follow-ups, and documentation.

- Focuses on client empowerment evidence-based or adopted best practices in providing treatment, seeking to assist clients to build on strengths in addressing treatment needs.
- Facilitates with clients and collaborates with the treatment team to develop, review, and update treatment goals and referrals both internally and externally.
- Attends staffings with Nursing Homes, Court Services, schools, agencies, and others, as necessary.
- Monday-Friday (8:00am-4:00pm)
- Maintains regular contact with an active caseload- most of which is conducted in client’s homes and in the community.
- Provides direct services to clients including individual and group treatment. Provides Substance Abuse treatment for dual-diagnosed clients.
- Consult with the supervisor regarding the transfer/placement of a client to another level of service, as needed.
- Assists clients in obtaining public entitlements, housing, community resources, etc.
- Organizes and participates in treatment-related clinical conferences and staffings on assigned clients.
- Conducts crisis intervention/evaluation/referral to persons who present themselves at the program.
- Performs initial assessments of clients after they have been deemed appropriate for participation in the program.
- Provide education and direction to clients, family members, and/or significant others.
- Takes responsibility for training in and documenting clinical chart requirements to meet program and state standards.
- Assists in facilitating transportation of clients.
- Bachelor’s degree in a related field, or
- Bachelor’s in any field and two (2) years’ experience, or
- High School diploma or GED and five (5) years’ experience
- Must possess a valid driver’s license
