This posting is for use for those interested in being a paid Summer Camp Worker or Event Worker at Lake Park High School District #108. Should the Summer Camp, or event, you were assigned to work be canceled, no pay will be issued to you.
The School Code requires applicants seeking employment at a school in a position requiring direct involvement with children or students to authorize an employment history review as part of the application process. To meet this requirement, please visit Please note : you do NOT need to pay the $15.00 fee for the "Premium" option unless you want access to your completed form forever. The "FREE" option gives you access to your form for 14 days once it is completed (in case you want to print it out for your records). Prior to completing the information on the Verifent website, if you have worked in a position (current or previous) where you were in direct contact with children, please contact your previous and / or current employer(s) that you have worked for to confirm the person / email address the Verifent Verification Requests should be sent to. Once you are on the website, click the 'Let's Get Started Now' button in the upper-right hand corner and select the 'Employees" pop-up option. On the next screen, scroll down and select the 'Initiate Experience Verification' button. Click on "Choose Your Hiring Entity" and type in Lake Park (choose Lake Park Community High School District 108 and click "Save Hiring Entity). Click "Continue." Please note, if you did not have direct contact with children at your current and / or previous employers, you can disregard Step 3. on the attached handout from Verifent for those employers. For your position, we "DO NOT" need the "Certified Experience Form," so please click the radial button to "No." You will need to complete the Faith's Law Self-Disclosure Form. To further help you complete this process, I have attached a step-by-step handout from Verifent for your convenience. Please note that all required documents listed below (paper and electronic) need to be on file in the Human Resources Office before you may begin working. Summer Camp and Event Workers are prohibited from beginning any work at Lake Park until after they receive written confirmation from the program coach or coordinator that they may begin working. Please download and complete / sign the attached documents. You will need to bring these (along with any required supporting documents) with you to your fingerprinting appointment.
1) Lake Park High School District 108 Online Application (completed and submitted)
2) W-4 Forms (Federal and State) - see attached
3) Direct Deposit Form - Recommended / Requested (include blank check with "VOID" written on it, or a Direct Deposit Verification form from your bank listing the account and direct deposit routing number your pay will be sent to) - see attached
4) TalentEd Records Electronic Notification / Acknowledgements (view / sign / submit) for : Board Policy; Child Abuse / Mandated Reporter; Guidelines for Acceptable Use of Technology System; Professional Boundaries; and Emergency, Illness, Accident form (this form needs to be updated annually). Login information for completing these electronic documents will be given once you have submitted your online application and the Human Resources Office receives notification from the program coach or coordinator that you will be working.
5) CBI - Criminal Background / Fingerprinting process - completed by all applicants (Fingerprinting is done by appointment only at : Lake Park High School District #108 Administration Center, 590 S. Medinah Road, Roselle, IL). Please contact Terri Svendsen in our Human Resources Office at 630-295-5421 to receive your login information (see item 4 above) and to set up an appointment to be fingerprinted and drop off your required paper documents.
6) DCFS (Department of Children and Family Services) Background Check form - completed by all applicants (please print neatly) - see attached PLEASE NOTE : THE DCFS BACKGROUND CHECK PROCESS HAS BEEN UPDATED AND NOW INVOLVES YOU (OR YOUR PARENT OR GUARDIAN IF YOU ARE UNDER THE AGE OF 18) COMPLETING INFORMATION ON THEIR WEBSITE. DETAILS WILL BE EMAILED TO YOU (OR YOUR PARENT OR GUARDIAN) FROM THE DCFS ONCE WE SUBMIT INITIAL INFORMATION TO THEM. IF YOU ARE UNDER THE AGE OF 18, PLEASE LIST YOUR PARENT OR GUARDIAN'S EMAIL ADDRESS ON THE ATTACHED FORM (NOT YOUR OWN).
7) I-9 Employment Eligibility Verification form (only Section 1. on page 1 needs to be completed / signed by you; page 2 is completed by our Human Resources office) - see attached At the time of fingerprinting, you will need to bring your Social Security Card (required for payroll purposes) and one (1) valid government issued picture ID is required (Driver's License or State ID) .
