Job Title : Physical Therapy Assistant

Experience : New graduates accepted, SNF experience preferred
License : State license verification required
Certifications : 1 Step PPD or Quantiferon or proof of positive PPD and chest X-ray

- Complete prescreen, resume, and license verification attached upon application
- Three references
- County criminal background check (7 years for all counties lived and worked)
- Federal criminal background check (7 years or more)
- Multi-state criminal search (7 years or more)
- Nationwide sex offender search (7 years or more)

- ACHA (updated every 5 years)
- Good moral character affidavit
