Lake Pointe Rehab is looking for an outgoing, compassionate, upbeat candidate for our Social Services Designee role.
Social Services Designee has many different tasks associated with the role here at Lake Pointe including: MDS social service assessments in Point Click Care, ancillary services for all residents, discharge planning, equipment ordering for discharge, and any social service concerns that may arise for each and every resident
As with any other management position, other duties may come up as assigned by administrator.
Compensation based on experience, and will be up for discussion.
