- Develop and implement Person Centered Service Plans (PCSPs) based on participant needs, preferences, and goals in collaboration with clients, families, and staff.
- Coordinate and oversee day-to-day program activities to ensure they align with agency goals and participant needs.
- Monitor and document client progress, ensuring accurate and timely completion of required reports and records.
- Provide support and guidance to direct support staff, ensuring activities are delivered in a safe, engaging, and person-centered manner.
- Serve as a liaison between clients, families, and external parties, addressing concerns and ensuring effective communication.
- Facilitate participant assessments and reviews to ensure programs remain responsive and relevant to individual needs.
- Ensure compliance with agency policies, procedures, and all applicable local, state, and federal regulations.
- Collaborate with team members to develop innovative program strategies and enhance service delivery.
- Organize training sessions or resources for staff to improve understanding of participant needs and effective service delivery.
- Handle administrative duties related to program operations, including scheduling, resource allocation, and logistics.
- Perform other duties as assigned to support overall program success and agency initiatives.
Qualifications:
- DD certification required and at least one (1) year of experience working with individuals with developmental disabilities in a direct support role.
- Bachelor's Degree
Proficiency in Microsoft Office Suite and familiarity with electronic health records (EHR) systems.
Ability to work both independently and collaboratively in a team environment.
Effective verbal and written communication skills.
Excellent organizational, problem-solving, and multitasking skills.
Strong understanding of person-centered planning and inclusive practices.
Preferred Qualifications:
- Knowledge of developmental disabilities, community resources, and relevant regulations (e.g., Medicaid waivers, state requirements).
- Experience in program development, coordination, or oversight.
- Certification in CPR/First Aid or other relevant credentials.
Physical Requirements:
This role is largely sedentary, involving prolonged periods of sitting and extensive use of computer and office equipment.
