Job Summary
We are seeking a compassionate and dedicated Hospice Nurse to provide high-quality patient care to individuals with terminal illnesses. Responsibilities
- Conduct thorough patient assessments, including vital signs and medical history evaluations.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
- Provide education and support to patients and their families regarding disease processes, treatment options, and end-of-life care.
- Maintain accurate medical records and documentation of patient care activities.
- Monitor patient conditions regularly, adjusting care plans as necessary based on ongoing assessments.
- Offer emotional support and counseling to patients and families during difficult times.
- Coordinate with other healthcare providers to ensure continuity of care across various settings.
- Participate in interdisciplinary team meetings to discuss patient progress and modify care strategies.
Skills
- Proficient in conducting vital signs assessments and understanding their implications for patient health.
- Experience in pediatrics, geriatrics, or critical care settings is highly desirable.
- Strong diagnostic evaluation skills to identify changes in patient conditions promptly.
- Ability to maintain comprehensive medical records accurately and efficiently.
- Expertise in patient assessment techniques to ensure holistic care delivery.
- Familiarity with primary care practices and protocols relevant to hospice settings.
- Compassionate approach towards providing patient care that respects individual dignity and preferences.
- Knowledge of working within a Level I Trauma Center is an asset.

Job Type: Part-time
Pay: $50.00 - $60.00 per hour
