What are the responsibilities and job description for the Per Diem Social Worker for Hospice Care (Barstow) position at Lake View Hospice?
Responsibilities:
- Provide comprehensive medical social work services to patients receiving hospice care
- Conduct mental health counseling and crisis management for patients and their families
- Collaborate with interdisciplinary team members to develop and implement care plans
- Assess patients' psychosocial needs and provide appropriate interventions
- Conduct individual and group counseling sessions to address emotional and behavioral issues
- Assist patients in accessing community resources and support services
- Advocate for patients' rights and ensure their needs are met
- Provide education and support to patients and families regarding end-of-life care
Requirements:
- Bachelor's or Master's degree in Social Work or related field
- Valid state license or certification as a Medical Social Worker
- Experience working with seniors, hospice care, or assisted living preferred
- Knowledge of behavioral therapy techniques and trauma-informed care
- Strong communication and interpersonal skills
- Ability to assess complex situations and provide appropriate interventions
- Proficiency in case management and documentation
