What are the responsibilities and job description for the RN Clinical Care Coordinator position at Lake Woods Nursing & Rehabilitation Center?
We are in search of a
- RN Clinical Care Coordinator
- to supervise, provide and coordinate nursing care in compliance with facility and regulatory policies and procedures and to assess residents’ care needs.
- Primary functions :
- Interview residents, residents’ representatives and others with information related to the Minimum Data Set (MDS) assessment.
- Collect all data necessary to complete MDS forms and related documentation.
- Make appropriate referrals to other health care professionals.
- Participate and / or conduct interdisciplinary meetings.
- Supervise and participate in the overall care of the residents
- Oversee clinical coordination, review labs, assist the physician with rounds
- Participate in the facilities Quality Assurance process to review facility systems as directed
- Wage Information :
- 12,000 SIGN ON BONUS
- 41.95 / hour -$46.45 / hour based on experience ad an RN & LPN\
- Including $0.85 / hour MI pass through pay until 09.30.25
