What are the responsibilities and job description for the MDS Nurse position at Lake Worth Rehabilitation Center?
As an MDS Coordinator, you will be responsible for developing, implementing and coordinating the Resident Assessment Instrument (RAI) process, ensuring the center stays in compliance with current professional practice standards, physician’s orders, company policies and procedures, and local, state, and federal regulations. Qualifications:
- Active, unrestricted Licensed Practical Nurse LPN or Registered Nurse RN licensure in the state of practice required
- Working knowledge of Medicare and Medicaid programs, LTC payer systems, the Omnibus Budget Reconciliation Act (OBRA), the RAI process, and the MDS process preferred OR prior nursing in a long-term care facility
Responsibilities
- Conduct comprehensive assessments of patients to develop individualized care plans.
- Ensure accurate documentation of medical records in compliance with HIPAA regulations.
- Collaborate with interdisciplinary teams to coordinate patient care and discharge planning.
- Utilize EHR systems to manage patient information and track progress.
- Stay updated on Medicare guidelines and coding practices to ensure proper billing and reimbursement.
- Provide education and support to patients and families regarding health management and care options.
