Job Summary
The MDS Coordinator is responsible for coordinating and completing the Minimum Data Set (MDS) assessments for residents in a skilled nursing facility (SNF). The coordinator will work closely with the nursing staff to ensure accurate and timely completion of the MDS assessments.
Responsibilities
- Coordinate and complete MDS assessments for residents in a SNF
- Ensure accuracy and completeness of MDS assessments
- Work closely with nursing staff to gather necessary information for MDS assessments
- Ensure compliance with state and federal regulations related to MDS assessments
- Participate in care planning meetings with nursing staff and other healthcare professionals
- Provide education and training to nursing staff on MDS assessments and related regulations
- Participate in quality improvement initiatives related to MDS assessments and resident care
Requirements
- Current RN/LPN license in the state of Kansas
- Minimum of 2 years of experience in a SNF setting
- Experience with MDS assessments and related regulations
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong attention to detail and organizational skills
