What are the responsibilities and job description for the Residential Nursing Coordinator position at Lakes Homes and Program Development?
This nursing position will oversee the medical needs of individuals who live in community residential support homes, that are licensed under Minnesota department of human services statutes 245D. Lakes Homes is corporate foster care, providing program services to adult with developmental disabilities and related conditions in homes of 3-4 individuals. We have six homes in Fergus Falls that a nurse would work with.
As a nurse for Lakes Homes, you will complete in home medical assessments, feel enthusiastic about guiding unlicensed staff to follow through with nursing orders, or coordinate clinic and hospital appointments. You will accompany individuals to appointments, complete medication/treatment orders and instruct staff in new medication or procedures for individuals and communicate with guardians and county case managers regarding health concerns.
You will oversee any medical cares and medication protocals in the homes. You will teach direct care staff, (DSP's) to be competent at giving medications and performing prescribed treatments. You will complete observed skills assessments to ensure that direct care staff demonstrate the ability to perform procedures consistent with Lakes Homes' policies and procedures, including but not limited to Blood pressures, home physical therapy, glucometer checks, ear care, nail care, peri care, AFO's TED socks, hearing aids, dentures and splints, reporting incidents and documenting narrative logs and Medication Administration records, documentation is completed and filed in personnel training files,
You will attend and participate in team meetings, aka care conferences and regular Lakes Homes staff meetings.
You will be responsible with communicating with pharmacies regarding new medications or medication changes, as well as the routine filling of prescribed medications. You will work with medical supply providers for supplies and equipment needed for the day to day cares of the individuals who need them.
You will work with other nursing personnel at Lakes Homes, but will be the primary nurse for the Fergus Falls Homes. If you are a LPN, you will work under the supervision of an RN within Lakes Homes nursing department and the executive administrator. If you are an R.N. You will have autonomy to work within your licensing parameters without direct supervision within the nursing department; your direct supervisor will be the executive administrator.

Pay: $28.00 - $32.00 per hour
