Position Summary:

Lakes Region VNA and Hospice is seeking a Home Care Manager who will be responsible for overseeing the delivery of high-quality home health care services to patients. This position ensures the coordination and management of care plans, supervises nursing staff, and collaborates with multidisciplinary teams to achieve optimal patient outcomes. The Home Care Manager will also play a crucial role in maintaining compliance with regulatory standards and fostering a positive working environment.


Key Responsibilities:

  • Patient Care Coordination:
  • Develop and oversee individualized care plans.
  • Ensure the timely and effective delivery of home health care services.
  • Collaborate with physicians, therapists, and other healthcare professionals.
  • Compliance and Quality Assurance:
  • Ensure compliance with state and federal regulations.
  • Monitor and maintain high standards of patient care and safety.
  • Conduct regular quality assurance audits and implement improvement plans.
  • Leadership and Supervision
  • Provide leadership and guidance to home health care nursing staff.
  • Conduct regular team meetings and training sessions.
  • Monitor performance and conduct performance evaluations.
  • Provides clinical support of the Hospice team when needed.
  • Administrative Duties:
  • Manage the scheduling of nurses.
  • Handle patient intake and discharge planning.
  • Maintain accurate and up-to-date patient records.
  • Community Engagement:
  • Serves as a liaison between the visiting nurse association and the community.
  • Participate in community outreach and educational programs.


Qualifications:

  • Education: bachelor’s degree in nursing or related field, master’s degree preferred.
  • License: Registered Nurse (RN) with a valid nursing license.
  • Experience: Minimum five years of experience in a leadership position in home health care or a related field.
  • Skills:
  • Strong communication and interpersonal skills.
  • Ability to work effectively with staff.
  • Excellent organizational and time management skills.
  • Knowledge of state and federal home health care regulations.
  • Proven leadership and management skills.
  • Ability to work independently and take initiative, be highly organized and pay strong attention to detail and accuracy; able to multitask and function well in a fast-paced environment.
  • Must be self-motivated, dedicated, detail oriented.
