Position Summary :
Lakes Region VNA and Hospice is seeking a compassionate and dedicated Social Worker to join our team. The Social Worker will support both Hospice and Home Care patients and their families. This role involves assessing the psychosocial needs of patients, developing care plans, and offering emotional support to help patients and their families navigate the complexities of healthcare. The ideal candidate will have a master's degree in social work and at least one year of healthcare experience.
Key Responsibilities :
Psychosocial Assessment : Conduct comprehensive assessments of patients' psychosocial needs, strengths, and challenges. Identify necessary interventions and resources to support the patient and family.Care Plan Development : Collaborate with the interdisciplinary team to create and implement individualized care plans. Ensure that social, emotional, and financial needs are addressed.Patient and Family Support : Provide counseling and support to patients and their families to help them cope with illness, loss, and end-of-life issues. Offer guidance on advanced care planning and decision-making.Resource Coordination : Assist patients and families in accessing community resources, including financial assistance, support groups, and other services. Facilitate referrals to appropriate agencies and organizations.Crisis Intervention : Respond to emergency situations and provide crisis intervention as needed. Address urgent needs related to safety, housing, and mental health.Documentation : Maintain accurate and timely documentation of all interactions and care plans in the patient's medical record. Ensure compliance with regulatory and organizational standards.Team Collaboration : Participate in interdisciplinary team meetings to discuss patient care and progress. Work closely with nurses, physicians, and other healthcare professionals to ensure comprehensive care.Education and Advocacy : Educate patients and families on their rights and available services. Advocate for the patient's needs within the healthcare system.End-of-Life Care : Provide specialized support to hospice patients and their families, focusing on comfort, dignity, and quality of life. Assist in the bereavement process as needed.Qualifications :
Education : Bachelor's Degree at minimum, Master's Degree in Social Work (MSW) from an accredited institution preferred.Experience : Minimum of one year of experience in healthcare, preferably in a hospice or home care setting.Licensure : Current and valid Social Worker license in the state of New Hampshire.Skills :Effective communication and interpersonal skills.
Ability to work effectively with diverse populations.Excellent organizational and time management skills.Knowledge of community resources and social services.Proficient in electronic medical records (EMR) and documentation.Working Conditions :
Environment : Work will be conducted in patients' homes, hospice care settings, and the community.Physical Requirements : Must be able to travel within the service area, perform tasks involving physical activity, and manage stressful situations.Schedule : Part-time or Per Diem with occasional on-call responsibilities.3 days, 20-24hrs / week
