Description
Under the direction and supervision of a Registered Nurse, the IDD Provider (IDDP) Licensed Vocational Nurse (LVN) will practice within the scope of the Texas Nurse Practice Act and Nursing Standards of Care of Professional Performance. The LVN is responsible for screening, organizing, implementing, monitoring, and delivering an array of health services to individuals served by the IDDP Waiver Services. The LVN assist with the development and maintenance of nursing service objectives and implements treatment and training as listed in the individuals Implementation Plan. The LVN functions as a liaison between the individuals, their families, staff, and the public-responding to inquiries and providing information on nursing care and services. This position may do focus assessments of prospective individuals to ensure that staff can provide the care needed. The LVN assures that there is compliance with the regulations pertaining to care plans and individuals in care. Required to perform and document on at least five (5) hours of direct time daily with the assigned caseload.
Salary: $15.64-$21.00/hrly
Work Schedule: 8-5pm Flex Monday-Friday
Salary: Depending on Qualifications
Monthly On-Call Stipend: $100.00

Minimum Qualifications
- High School Diploma
- License Vocational Nurse license from the State of Texas
- Knowledge of community-based care.
- Experience working with individuals with mental health or IDD
- Familiarity with HCS/TXHML/ICF, TAC, and billing guidelines.
