What are the responsibilities and job description for the Mental Health Caseworker/QMHP (ACT Team) position at Lakes Regional Community Center?
Responsibilities includes assisting individuals with gaining access to medical, social, educational, and other needed resources, including provision of rehabilitative and skills training, case management, care coordination, treatment planning, crisis services, as well as documentation of client progress. Also provides supported employment and telemedicine services. Must have a valid Texas driver's license and acceptable driving record, as well as personal liability insurance as required by the state of Texas. Working computer skills including Microsoft Office.
