We are a culturally responsive Community Health Center looking for behavioral health counselors to increase equity of service delivery to our Hmong and Spanish-speaking patients.

Providing care in a patient's primary and preferred language, and having a deeper understanding of their cultural beliefs, and appreciating their experience as someone who speaks another language can facilitate deeper therapeutic exploration and help ensure the best outcomes for our patients.

The Behavioral Health Counselor is responsible for identifying, triaging, and managing medically underserved patients within the integrated primary and behavioral health care setting, while working with service agencies that provide assessment, prevention, intervention, treatment, and related ancillary support services to people with alcohol & drug-related problems, mental illness, and/or co-occurring conditions. The employee will also provide services through ongoing psychotherapy, psycho-education, warm hands-off approaches and/or community outreach. While focusing on substance abuse difficulties, the goal will be to help patients overcome addiction by providing them with the skills and education they need.

ESSENTIAL DUTIES, TASKS, & FUNCTIONS

· Assesses the clinical status and therapeutic needs of underserved patients with mental illness, substance abuse, and chronic diseases through ongoing psychotherapy and/or brief consultative sessions in an integrated service model.

· Assesses all new intakes to determine presence & extent of substance abuse issues and completes the evaluation report.

· Maintains collaboration with the interdisciplinary team in recognizing, addressing, and referring patients with mental illness, substance abuse and psychosocial problems efficiently and effectively to prevent further psychological or medical deterioration.

· Documents clinical activity presenting diagnostic & interview findings, clinical evaluations, psychosocial assessments, and recommendations.

· Acts as a case manager by coordinating services for clients, ensuring continuity of care & the client’s ability to receive appropriate types of service.

· Formulates, implements, evaluates, and reviews treatment plans & DSM-IV diagnoses.

· Educates patients & their support systems on dealing with, supporting, and understanding the patient’s abuse, and providing prevention & treatment enhancement techniques.

· Maintains an electronic medical record that accurately reflects the services provided and the patient’s growth toward treatment goals.

· Monitors patient progress and makes changes to the therapeutic approach and best practices, based on patient’s response to treatment.

· Knowledge of the DSM-IV criteria & demonstrates the ability to diagnose & apply diagnoses for treatment planning.

· Strong desire to assist people suffering from a drug or alcohol abuse program.

· Ability to gain patient’s confidence, trust, and respect while maintaining an emotional distance.

· Has the ability to work independently, effectively manage time, possess good judgement, and maintain organizational skills while possessing strong oral & written communication skills.

· Will establish and maintain productive and professional relationships with patients, staff, and community partners.

· Possesses creativity in identifying problems and creating solutions.

· Ability to handle confrontational situations in a professional and productive manner.

· Knowledge and compliance of all applicable guidelines, policies, procedures, and laws.

· Keeps all matters related to the organization confidential in accordance with HIPAA and the confidentiality policies and procedures.

EDUCATION & TRAINING REQUIREMENTS

· Requires a Master’s degree in Social Work, Psychology or other related field

· Current WI licensure as an LPC, LCSW, LMFT, or Licensed Psychologist

· Must hold a SAC or CSAC credential in good standing

· Current CPR/BLS Certification upon hire or willing to obtain & renew as needed.

Job Type: Full-time

Pay: $64,800.00 - $81,300.00 per year
