This position offers a unique, hybrid schedule where you'll work in our Sheboygan Clinic located at 1721 Saemann Ave as well as part of our medical team within a local elementary school, providing primary medical care to those students, teachers, and their family members.
As a Nurse Practitioner at LCHC, you will:
· Deliver Compassionate Care: Make a meaningful difference by serving a diverse patient population, including many from underserved communities.
· Collaborate in an Integrated Care Model: Work as part of a dynamic, interdisciplinary team that includes Behavioral Health, Nursing, Social Work, Pharmacy, and Dental providers, all supporting your efforts to deliver comprehensive care.
· Shape the Future of Healthcare: Engage with and mentor students training at LCHC, fostering the next generation of healthcare professionals.
· Inspire and Lead: Guide and motivate team members to achieve their best while fostering a collaborative and supportive work environment.
