For consideration, you must live in Colorado as you will be making 20 – 30 customer / client visits per month.
This position pays $22.07 / hr and offers a performance-based pay increase after 6 months, up to $24.52 / hr. Hours are 40 hours a week, Monday – Friday.
The ideal candidate would have at least 1 of the following :
Bilingual in Spanish / English
Experience with Intellectual and Developemental Disabilities
Hospital Experience
Nursing Facility Experience

Job Highlights : This role is the first eligibility step to see if a person qualifies for long-term services and support. This position helps the member understand the process and steps needed to qualify for services and communicates outcomes related to eligibility. The intake case manager assists the members to determine the best services to meet their individual needs.
As a Case Manager, you’ll be at the heart of transforming lives and providing vital support to those in need. Your responsibilities will include :
Conducting thorough assessments to identify client needs and preferences, using a compassionate and personalized approach.
Collaborating with clients and their families to establish tailored care plans and achievable goals.
Facilitating care coordination and advocating for clients by connecting them with essential services and resources.
Educating clients on available benefits, programs, and services, empowering them to make informed decisions.
Monitoring client progress, ensuring satisfaction, and adjusting care plans as necessary for optimal outcomes.
Representing clients’ interests in meetings and assisting with complaint resolution processes.
Qualifications :
A bachelor’s degree in a related field OR five years of relevant experience, showcasing your dedication to human services or direct care.
Residency in or around Denver, Colorado, with a willingness to work remotely and engage with the community.
Commitment to safety and compliance, including background checks, and a clean driving record.
Possession of a valid driver’s license, motor vehicle insurance, and a reliable vehicle for work-related travel.
