Lakeshore Talent is seeking a Occupational Therapist (OT) for a direct-hire opportunity in Sacramento, CA!
Location : Sacramento, CA
Pay : Salary : $119,000 – $124,800 (DOE)
Schedule : Full-time, 40 hours / week between 8-5
Work Split : 70% at center, 30% in-home
Mileage Reimbursement : Provided
Requirements :
- Education : Graduate of an Occupational Therapy curriculum accredited by the American Occupational Therapy Association (AOTA) and the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Degree : Bachelor’s, Master’s (MOT), or Doctorate (OTD) in Occupational Therapy.
- Certification : National Certification by AOTA or eligibility for the National Registration Examination.
- Licensing : Registered and licensed in the practicing state.
- Other : Valid driver’s license, reliable transportation, good driving record, and auto insurance as required by law.
- Experience : Minimum 1 year of experience working with frail or elderly populations.
- Preferred : 2 years of experience, including 1 year in a geriatric setting.
- Software Proficiency : EPIC (EMR), Outlook (email), MS Teams (team communication & meetings).
Scope of Work :
The Occupational Therapist (OT) provides evaluations and treatment plans to participants, ensuring care aligns with organization’s policies and state regulations. Key responsibilities include :
Performing clinical occupational therapy evaluations and treatment at acute, restorative, and maintenance levels.Assessing participant conditions, developing treatment plans, and communicating with participants and caregivers.Integrating occupational therapy treatment into the team Plan of Care (POC).Assisting with ordering, distributing, and maintaining durable medical equipment (DME).Conducting home visits and educational sessions as needed, documenting activities and participant responses.Educating participants and caregivers on safety, ADLs, and appropriate use of DME.Administering treatment programs as per interdisciplinary team (IDT) assessments.Instructing caregivers, family members, and staff on maximizing functional abilities and documenting progress.Evaluating charts, tracking participant progress, and maintaining necessary records.Assisting physicians and care teams in developing and updating care plans.Participating in in-service programs, interdisciplinary team meetings, and staff training.Completing patient evaluations, clinical notes, and progress reports timely and accurately.Reporting patient incidents, variances, or complaints promptly.Ensuring compliance with safety, infection control, and quality assurance programs.Attending daily team meetings and supporting care coordination.Benefits (Effective 1st of the Month Following Hire) :
