Salary: $92,872 - $102,648
Location: Orlando, FL
The Physical Therapist provides clinical physical therapy evaluations and treatment plans for program participants (ages 55 and older) on an acute restorative or maintenance level as needed. They integrate physical therapy treatment plans into the Plan of Care (POC) as directed by physician orders. All participant care is provided in accordance with InnovAge policies and state regulations.
Responsibilities include:
- Screen and evaluate participants for rehabilitation needs and follow APTA ethical guidelines
- Collaborate with nursing home and assisted living staff regarding rehabilitation needs
- Refer participants to Occupational Therapy (OT) and Speech Therapy (ST) as needed
- Communicate and document changes in POC and ensure staff and family members are informed
- Provide physical therapy treatments, including modality treatment, therapeutic exercises, and functional mobility training
- Provide written instructions and illustrations for participants and caregivers
- Ensure participant and caregiver understanding of the Durable Medical Equipment (DME) loan program
- Delegate therapy plans to Physical Therapist Assistants and Rehab Aides as appropriate
- Instruct participants and caregivers on equipment usage, mobility, and safety
- Make home visits and provide educational sessions as needed
- Actively participate in interdisciplinary team meetings and planning
- Conduct peer reviews and participate in Quality Management (QM) activities
- Document all procedures in participant’s medical chart in a timely manner
- Order, inventory, distribute, and maintain DME for participants
- Train and educate contract physical therapists on InnovAge PACE systems
- Identify and resolve issues with DME promptly
- Assist in budgeting and procurement within the rehab department
- Provide training to non-rehab staff (e.g., CNAs, Drivers) regarding participant needs
- Participate in interdisciplinary teams to develop and monitor Plans of Care (POC)
Required Qualifications:
- Bachelor’s Degree in Physical Therapy
- Completion of an accredited physical therapy program approved by APTA, CMS, or other relevant organizations
- Active Physical Therapist license issued by the State of Florida
- Minimum of one (1) year of experience working with the frail or elderly
- Reliable vehicle
- CPR/BLS Certification and First Aid Certification (ARC or AHA)
- Preferred Qualifications:Two (2) or more years as a practicing Physical Therapist
