Lakeshore Talent is in search of an RN Manager Home Care for a direct hire opportunity at two locations in Philadelphia, PA. This role offers a salary of $96,000 – $115,000 and an approved $7,500 sign-on bonus. The selected candidate will begin immediately.

Position Details :

  • Schedule : Monday – Friday, 8 AM – 5 PM (On-Site)
  • Software : Epic EMR

Key Responsibilities :

  • Lead and manage a team of RNs, Schedulers, Medical Records personnel, and CNAs.
  • Oversee both field-based clinical staff and office-based operational staff.
  • Ensure efficient staffing, scheduling, and quality of care for home care participants.
  • Conduct hiring, training, performance evaluations, and employee development.
  • Monitor and manage departmental expenditures, including payroll and overtime.
  • Serve as a primary participant in the Interdisciplinary Team (IDT) for collaborative care planning.
  • Conduct audits and ensure compliance with organizational policies and regulatory standards.
  • Perform initial and annual assessments of participants in their home environment.
  • Develop and coordinate Plans of Care (POC) with scheduling teams and caregivers.
  • Administer medications, monitor compliance, and provide health counseling.
  • Ensure all medical records, reports, and participant data are accurately maintained.
  • Required Qualifications :

  • Active RN license (as required by law).
  • Bachelor of Science in Nursing (BSN).
  • 2 years of RN healthcare experience, preferably with geriatrics.
  • Leadership experience, including staff supervision and scheduling.
  • Current First Aid and BLS certifications (American Heart Association or Red Cross accepted).
  • Valid driver’s license and auto insurance.
  • Preferred Experience :

  • Home Health, Home Care, Hospice, SNF, or LTC experience.
  • Ability to work in a complex matrix leadership environment.
  • Strong critical thinking, emotional intelligence, and collaboration skills.
