Lakeshore Talent is in search of an RN Manager Home Care for a direct hire opportunity at two locations in Philadelphia, PA. This role offers a salary of $96,000 – $115,000 and an approved $7,500 sign-on bonus. The selected candidate will begin immediately.
Position Details :
Schedule : Monday – Friday, 8 AM – 5 PM (On-Site)
Software : Epic EMR
Key Responsibilities :
Lead and manage a team of RNs, Schedulers, Medical Records personnel, and CNAs.
Oversee both field-based clinical staff and office-based operational staff.
Ensure efficient staffing, scheduling, and quality of care for home care participants.
Conduct hiring, training, performance evaluations, and employee development.
Monitor and manage departmental expenditures, including payroll and overtime.
Serve as a primary participant in the Interdisciplinary Team (IDT) for collaborative care planning.
Conduct audits and ensure compliance with organizational policies and regulatory standards.
Perform initial and annual assessments of participants in their home environment.
Develop and coordinate Plans of Care (POC) with scheduling teams and caregivers.
Administer medications, monitor compliance, and provide health counseling.
Ensure all medical records, reports, and participant data are accurately maintained.
Required Qualifications :
Active RN license (as required by law).
Bachelor of Science in Nursing (BSN).
2 years of RN healthcare experience, preferably with geriatrics.
Leadership experience, including staff supervision and scheduling.
Current First Aid and BLS certifications (American Heart Association or Red Cross accepted).
Valid driver’s license and auto insurance.
Preferred Experience :
Home Health, Home Care, Hospice, SNF, or LTC experience.
Ability to work in a complex matrix leadership environment.
Strong critical thinking, emotional intelligence, and collaboration skills.
