What are the responsibilities and job description for the Medical Provider LPN/CMT position at Lakeview Behavioral Health?
Lakeview is looking for an experienced LPN/CMT to join out team. This position is part time 20hrs a week. days and shifts will very between Monday-Friday 8am-4:30pm at our Grand Rapids location.
We are looking to have this position start July 1, 2025.
The Medical Provider - LPN/CMT will be responsible for providing medical treatment and services for co-occurring disorders. The LPN/CMT provides analysis of work processes to improve outcomes and services to meet information needs and drive improvement activities. Facilitates the review and interpretations of state and regulatory standards and reporting requirements; participates in program planning and attends appropriate meetings to gain and share necessary information with CCO and organizational leadership.
Qualifications:
- Be currently licensed and in good standing as a Licensed Practical Nurse (LPN) or a Certified Medication Technician (CMT).
- Be 18 years of age.
- Ability to attain a working knowledge of 245G and other applicable rules and statutes.
- Ability to successfully complete MN Dept of Human Services background study.
- Must have or obtain 12 hours of training in co-occurring mental health problems training within six months of hire.
Essential duties and responsibilities:
- Have a working knowledge and understanding of all policy and procedures.
- Provide services within the scope of practice for the discipline in which licensure/certification is active.
- Offer medical services to clients including but not limited to: health care services, nursing services,, emergency services, services necessary for medical intervention, or consultation with medical professionals that are documented as being available and offered by the provider.
- Participate in service initiation, initial service plan creation addressing the client’s immediate health, safety, and preliminary service needs, evaluation, individual treatment plan creation using a person-centered approach, individual and group counseling, individual and group education, treatment coordination, collateral meetings, and discharge planning by addressing medical needs at all points during treatment.
- Maintain and update policies, procedures, and descriptions related to medical and health care services, medical emergencies, medical records, and interventions available and provided through the organization and obtain physician signatures for those procedures that require them.
- Help create a positive, upbeat work environment; participate in team building activities; exemplify a positive manner and attitude and encourage others to do the same by example; inform the CCO of personnel issues that need intervention at a higher level.
- Adhere to Lakeview Behavioral Health policy and procedures.
- Complete all documentation necessary in the time required to be in compliance with statutes, policy and procedures which may include but not be limited to weekly treatment plan reviews/progress notes, discharges, treatment coordination notes, and contact case notes.
- Ensure that the medical needs of the client and the nature and provision of any medical services provided were documented in the client file when medical services are delivered.
- Support APRN’s/RN’s as needed with medication refills and completing prior authorizations.
- Participate in the weekly clinic meetings to review and update client progress notes. Be an active member of a clinical team and work together with the collaborating professionals both within and outside of the agency according to statutes, policy and procedures.
- Participate in community education and events as asked.
Continuing Education and Training
- Participate in orientation within 24 hours of first providing direct contact services and as required thereafter on the reporting requirements and definitions specified under Minnesota Statutes, sections 626.557 and 626.5572, the requirements of Minnesota Statutes, section 245A.65, the license holder's program abuse prevention plan, and all internal policies and procedures related to the prevention and reporting of maltreatment of individuals receiving services, specific job-related duties, client rights and needs, HIV minimum standards, grievance procedures, ethics, client confidentiality, and the drug and alcohol policy.
- Stay current regarding professional research, techniques, etc.
- Complete continuing education courses as required by the State of Minnesota to maintain licensure/certification.
- Complete continuing education required by Lakeview Behavioral Health’s policy and procedures and additional training asked to attend.

Scope of Practice
According to the Nurse Practice Act - Minnesota Statute Section 148.171, The "practice of professional nursing" means the performance, with or without compensation of those services that incorporates caring for all patients in all settings through nursing standards recognized by the board and includes, but is not limited to:
- (1) providing a comprehensive assessment of the health status of a patient through the collection, analysis, and synthesis of data used to establish a health status baseline and plan of care, and address changes in a patient's condition;
- (2) collaborating with the health care team to develop and coordinate an integrated plan of care;
- (3) developing nursing interventions to be integrated with the plan of care;
- (4) implementing nursing care through the execution of independent nursing interventions;
- (5) implementing interventions that are delegated, ordered, or prescribed by a licensed health care provider;
- (6) delegating nursing tasks or assigning nursing activities to implement the plan of care;
- (7) providing safe and effective nursing care;
- (8) promoting a safe and therapeutic environment;
- (9) advocating for the best interests of individual patients;
- (10) evaluating responses to interventions and the effectiveness of the plan of care;
- (11) collaborating and coordinating with other health care professionals in the management and implementation of care within and across care settings and communities;
- (12) providing health promotion, disease prevention, care coordination, and case finding;
- (13) designing and implementing teaching plans based on patient need, and evaluating their effectiveness;
- (14) participating in the development of health care policies, procedures, and systems;
- (15) managing, supervising, and evaluating the practice of nursing;
- (16) teaching the theory and practice of nursing; and
- (17) accountability for the quality of care delivered, recognizing the limits of knowledge and experience; addressing situations beyond the nurse's competency; and performing to the level of education, knowledge, and skill ordinarily expected of an individual who has completed an approved professional nursing education program as described in section 148.211, subdivision 1.
CMT Scope of Practice (Allowed Duties):
- Administer oral, topical, eye, ear, nasal, and inhalant medications as directed by a licensed nurse.
- Measure and document vital signs before and after medication administration if required.
- Document medication administration accurately in the medication administration record (MAR).
- Observe and report side effects or changes in patient condition to the supervising nurse.
- Assist with PRN medications, but only under the direct supervision and direction of a nurse (and with proper documentation and assessment).
- Work under the delegation and supervision of a Registered Nurse (RN) or Licensed Practical Nurse (LPN)
Wage Range depending on qualifications and experience:
LPN: $23-$27/hr
CMT: $15-$22/hr
