About the Department
This role supports an autonomous department that provides in-home medical services to individuals, delivered by our highly skilled homecare and hospice nursing staff. It is essential to adhere to the guidelines established by homecare and hospice licensing agencies, as well as the Centers for Medicare & Medicaid Services (CMS).
Mission for Position
Supports the mission and goals of Lakewood Health System through personal and professional conduct including concern demonstrated for all people served, regardless of age, race, diagnosis, or economic background. Is an active member of the Lakewood Health System team by contribution of ideas, suggestions, regular work, attendance, punctuality, adherence to Lakewood Health System rules, policies and patient confidentiality.
Duties & Responsibilities
- Completes preadmission process for new admissions to home care and hospice.
- Initiates and maintains charts for admissions to home care and hospice.
- Maintains specific forms for regulatory compliance.
- Answers departmental phones and directs call and/or information to appropriate individuals.
- Assists with tracking and faxing of physician orders.
Position Type
- Monday Friday 8:00am 4:30pm, no weekend or holiday rotation.
Qualifications
- High School Diploma or Equivalent
- Prior Office Experience Preferred but not Required
- Current, Valid MN Drivers License
