About the Home Health/Hospice Team
Our home health/hospice team prides itself on providing top quality care to our patients in their own home following hospital discharge, change in health condition, or transition to hospice services.
Mission for Position
Provide skilled nursing care and general health guidance to patients in their home care or hospice setting.
Duties & Responsibilities
- Correctly implement the client’ Plan of Care established by the physician.
- Observe proper infection control and safety procedures.
- Supervise health assistant services.
- Coordinate care team to meet patient needs.
Position Type
- This is a full-time position with a schedule of Monday–Friday, 8:00 AM–4:30 PM. This role includes one holiday per year and participation in a call rotation, covering one evening every other week and approximately every eighth weekend.

Qualifications
- Current MN RN licensure.
- Current, valid MN Driver's License and current auto insurance.
