What You will Do:
- Work closely with MDS and clinical resources to achieve set goals
- Develop improving treatment and care plans
- Evaluate patients care that both skilled and LTC patients receive
- Meet with clinical staff and families to discuss patient's condition and treatment plan
- Attend and host meetings to discuss patients care with IDT team
- Assist DON with duties as assigned
What You will Need:
- RN (Registered Nurse) license active in the state of CO
- 1-3 years of MDS experience
- Self-directed
- Self-motivated
- Detail Oriented
- Display ability to manage multiple projects
- Ability to successfully instruct and coach staff
- Excellent customer service skills
