Social Services Director
Status: Full-Time
Pay: $24 - $27/HR Depending on experience
Shift: Monday-Friday
What You’ll Do:
· Provides counseling as requested to residents and their families.
· Discharge Planning
· Grievance Log/Follow up as needed
· Forecast and prepare the department’s budget for equipment, supplies and labor and submit to the Administrator for review, recommendations and approval
· Arrange for medically related ancillaries as indicated
· Complete resident assessments and attend care planning meetings in accordance with Omnibus Budget Reconciliation Act (OBRA) guidelines.
· Interview residents or family members as necessary to obtain social history
What You’ll Need:
- 1 year experience (preferred) in long-term/rehab setting
- License: Certified Social Worker
- Must possess, as a minimum, a minimum of a bachelor’s degree in social work or A human services field including sociology, gerontology, special education, rehabilitation counseling, psychology, etc.
